PrimeStar Platinum Enrollment Instructions - Three Ways to Enroll

1.  Apply Online.  Enrollment is available online by clicking on the link below. 

2.  Print and Mail in your Application.  Please mail to the address below prior to the 20th of the month for a first of the following month effective date (the effective date is the first of the month following the day in which the application is received by PrimeStar):

3.  Fax in your Application.  For your convenience, PrimeStar will accept faxed copies of your application.  You must choose Credit Card or ACH payment options.  Please fax your enrollment application to:  425-821-3587.
