| The convenient monthly payment option is available when you pre-authorize a monthly | ||||||

| If you don't choose to pay by this method, the only other option is the annual premium. | ||||||
| You have ten days after you submit payment to be sure you are completely satisfied. | ||||||
| If, within these ten days, you decide to cancel, you may request a full refund provided | ||||||
| you have not utilized the plan. If you later decide to cancel your membership within | ||||||
| the first year, you will be subject to a $50.00 cancellation fee and will receive a | ||||||
