Health Plans for Individuals and Families                       

LifeWise offers a variety of individual and family health plans designed to meet your specific needs. Their plans focus on prevention, choice and value.  Health plans can be divided into three categories: WiseAdvantage (Comprehensive), WiseEssentials Rx, WiseEssentials Copay & WiseSimplicity (Catastrophic) and WiseSavings (HSAs).

WiseAdvantage (Comprehensive):   This plan provides you with the most coverage (including pharmacy, vision care and maternity).  Preventive Care Exams, Screenings (includes mammograms) and Immunizations are covered in full (no deductible or coinsurance). These plans also include $30 office visit copays - deductible waived! These plans are designed for those looking to cover all their bases.

WiseEssentials Rx & WiseEssentials Copay (Catastrophic):  High deductible plans designed for those looking to cover major medical. The WiseEssentials Rx plans offer 6 annual office visits paid at 75% (deductible waived) and coverage for generic prescriptions - $15 copay!  WiseEssentials Copay is similar to WiseEssentials Rx, but offers higher annual deductibles and covers the first 3 office visits with a $25 copay, and does not have prescription coverage.  Both plans cover Preventive Care Exams, Screenings (included mammograms) and Immunizations in full - no deductible or coinsurance!

WiseSimplicity (Catastrophic):  This plan offers the highest deductible and is designed for those looking to keep their monthly rate low and are willing to pay more out-of-pocket.  Preventive Care is covered at 100% - deductible waived! 

WiseSavings (HSAs):     High deductible plan combined with an optional health savings account - tremendous tax benefits. Preventive care not subject to the deductible!
