Before the deductible, members receive: Four upfront office visits covered at 100% after a $35 copay $200 in upfront X-ray and lab benefits covered at 100% Preventive Care - Coinsurance only; no deductible or annual limit!  Asuris Emerge Plus offers the most benefits, including maternity, prescription and vision care.  There are four deductibles to choose from with the $1000 deductible plan being the only comprehensive option. Before the deductible, members receive: Four upfront office visits covered at 100% after a $25 copay $400 in upfront X-ray and lab benefits covered at 100% Prescription drugs ($10 copay for generics!) Preventive Care - Coinsurance only; no deductible or annual limit! Vision Care: Routine Eye Exam & Hardware - covered at 80% to $150 combined maximum PCY
